Standard email is not a secure way to send health information. Please do not include a patient's health information in an email. For referrals, use fax or our online referral form, or call us and we will tell you how to proceed.

If you contact us, we use the information you give us to respond to you, and for the referral or care coordination you ask about.

Online referral form

The "Send a referral online" button opens a form hosted by Jotform, a separate service, on its own website. Information you submit through that form is handled through Jotform's service and our own handling of referrals, and is not collected by this website. Please use the form for referral purposes only.

Health information

Health information that we receive or create in connection with a referral or with care is handled in accordance with applicable privacy laws, including HIPAA where it applies. This website notice describes only the website and does not replace any notice of privacy practices that we provide to patients.

This website links to other sites, such as Jotform. We are not responsible for the privacy practices of other sites, so please review their notices.
