Support for anxiety
and/or depression.
Emotional health belongs in the care plan. Our anxiety and/or depression program brings screening, regular outreach, care coordination, and connection to appropriate resources into the management of chronic illness.
Living with illness
affects more than
the body.
Managing a chronic condition can be stressful and isolating, for patients and for the people who care for them. When emotional health is part of the care plan, it is easier to notice when someone is struggling with anxiety or depression and to connect them with the right support.
This program brings that attention into the care patients are already receiving. Our team works alongside the patient's clinical team, and connects patients to appropriate resources when more support is needed.
Regular, caring contact
and clear next steps.
- Screening for anxiety and depression

- Care coordination with the patient's care team
- Connection to appropriate resources
- Support for caregivers and family members
If you or someone you know is in crisis or thinking about self-harm, call or text 988 (Suicide & Crisis Lifeline), or call 911 in an emergency. This program is not an emergency or crisis service.
Part of caring for
the whole person.
Support for anxiety and/or depression is offered as part of managing chronic illness. Programs are tailored to clinical needs, patient goals, eligibility, and the care already in place. Everything is delivered by telehealth, so there is no need to travel to an office, anywhere in Texas.
Coverage and out-of-pocket costs depend on the program, insurance benefits, and individual eligibility. Our team reviews available benefit information and discusses potential patient costs before enrollment.
