For patients & families

Does Trinity work with my current doctor?

Yes. Our approach is to support existing provider relationships and coordinate with the patient's clinical team. If primary care access is missing, we help identify an appropriate path to care.

Do I need to travel or come to an office?

No. Trinity's care is delivered entirely by telehealth, so no in-person visit is required, wherever you are in Texas.

Where does Trinity provide support?

Trinity delivers its programs by telehealth, so we can support patients anywhere in Texas, with no in-person visit required. Contact our team to confirm eligibility and next steps.

Can family members or caregivers be involved?

With the patient's permission, family members and caregivers can participate in care planning, education, and communication about the patient's needs and goals.

What will my program cost?

Coverage and out-of-pocket costs depend on the program, insurance benefits, and individual eligibility. Our team reviews available benefit information and discusses potential patient costs before enrollment.

Is palliative care the same as hospice?

Palliative care supports quality of life during serious illness and can accompany active treatment. Hospice is a distinct model with specific eligibility requirements. Trinity helps patients and families navigate appropriate support as their needs change, and if you choose hospice at a later date, we can help connect you with our community hospice partners. Learn more about palliative care.

How is my health information protected?

We handle health information in accordance with applicable privacy laws, including HIPAA where it applies. For your privacy, please do not send health information by standard email. Call us, fax, or use our online referral form, and our team will tell you how to proceed. You can read how this website handles information in our Privacy Notice.

For healthcare partners

Who can refer a patient?

Healthcare and community partners such as providers, hospital and discharge teams, skilled nursing and assisted living facilities, home health agencies, hospice organizations, churches and faith communities, and community outreach and support organizations can refer patients. Patients and family members can also contact us to discuss a need.

How do I send a referral?

You can use our online referral form, fax 800-915-0474, or call (817) 910-6711. If you are not sure what to send, our intake team will tell you what information we need. See how to refer.

What happens after a referral?

We review clinical needs, program fit, coverage, and the patient's existing care relationships. With patient consent, we establish goals and coordinate the next steps with the care team, then stay connected with ongoing outreach, education, and communication.

Can we set up a partnership meeting?

Yes. Email intake@trinitymwc.com to request one. Please do not include patient health information in the email.
